Renewal of Passport

REQUIREMENTS FOR RENEWAL TO E-PASSPORT

1. Personal appearance

2. Accomplished passport application form

3. Original passport and photocopy showing the passport holder’s name, back cover bearing the passport officer’s signature and recent visa stamp.

Additional requirement for holders of old brown passport: Original and one (1) photocopy of birth certificate issued by NSO, driver’s license, marriage certificate or baptismal certificate showing the applicant’s middle or maiden name)

Note: Passports should be renewed 6 months before the date of expiry / Processing period for passports is 3 to 4 weeks

Note: Passports unclaimed or uncollected after (6) six months from the date of issuance will be cancelled per DFA regulations (No.37-03)

Requirements for change in surname

By reason of marriage

  • Original and a photocopy of Marriage Certificate issued by NSO and authenticated by Department of Foreign Affairs  (if marriage took place in the Philippines)

By reason of divorce or annulment

  • Original and one photocopy of annotated Marriage Contract from NSO and authenticated by Department of Foreign Affairs (annulment in the Philippines)

  • Original and one photocopy of divorce decree with English translation and authenticated by the Ministry of Foreign Affairs of the country where the divorce was obtained (divorced whose marriage was not previously registered in the Philippines)

  • Original and one photocopy of annotated Report of Marriage from NSO and authenticated by Department of Foreign Affairs (divorced whose  marriage was previously registered in the Philippines)

By reason of death of spouse

  • Original and one photocopy of death certificate issued by NSO and authenticated by DFA (if death occurred in the Philippines)

  • Original and one photocopy of death certificate with English translation and authenticated by the Ministry of Foreign Affairs of the country where death certificate was issued (if death occurred outside the Philippines)
